Substitute Teacher Procedures
DISTRICT 128 maintains a list of substitutes who are called on an as-needed basis to substitute at Libertyville High School and Vernon Hills High School.  Effective 3/9/09, we are not accepting new applications for substitute teachers.  We will reopen the application process on 8/1/09 for the 2009-2010 school year.
In order to be placed on the substitute call list, the following items must be completed and submitted in person to our district office at 50 N. Lakeview Parkway, Suite 101, Vernon Hills, IL.
  • Official college transcripts reflecting all college/university coursework
  •  Photocopy (both sides) of substitute teaching certificate or teaching certificate (Must be registered in Lake County)
  • Evidence of a physical and negative TB test within 90 days of application
  • Employment Eligibility Verification Form (I-9) (Must provide in person 2 documents that establish identity and employment eligibility)
  • Criminal background fingerprint check (Fingerprinting will be scheduled after receipt of required substitute documents)
  • Teacher Retirement System (TRS) member information and beneficiary form
  • Social Security Form (SSA-1945)
  • State & Federal W-4 forms
  • Bloodborne Pathogen, Child Abuse & Neglect, Drug & Alcohol Abuse, Copyrighted Works Guidelines, Sexual Harassment and Internet Acceptable Use Policy Forms
  • Complete and submit our online application found on our web site at www.d128.org
After all application materials have been submitted, please contact our substitute coordinator at 847-327-7039 to schedule a short orientation session.
Substitute teacher daily rate:     1 – 19 days …………$95.00       20+ days …………….$100.00
