Leadership Workshop Registration
Option 1: Enter participants ONE-AT-A-TIME on a fill-in web form.
Simply fill in the information for each participant, click the "Finish" button at the bottom of the form and it'll take you back to enter more.
Option 2: Enter ALL your participants on an Excel spreadsheet and email it.
 Note:
This is NOT a fill-in form that you simply click a "submit" button to
send your entries. To use this form, you must have Microsoft Excel
installed on your computer.
Follow these steps carefully:
- Click HERE to access the entry form file.
- Choose to "Save to Disk" and save the file to your computer's desktop.
- Open the file with Microsoft Excel.
- Complete your entries. For phone numbers, simply enter numbers only without parentheses or dashes. They'll format automatically.
- After you're done, SAVE the completed file to your desktop and CLOSE out of Excel.
- Attach the completed file to an email addressed to mick.torres@d128.org (please include your school name in the "Subject" field.)
- I will email me you a verification of entries within 48 hours.

Sports Guidelines
