Leadership Workshop Registration
 

Option 1:  Enter participants ONE-AT-A-TIME on a fill-in web form (CLICK HERE.) 

Simply fill in the information for each participant, click the "Finish" button at the bottom of the form and it'll take you back to enter more.
 
Option 2:  Enter ALL your participants on an Excel spreadsheet and email it.
Note:This is NOT a fill-in form that you simply click a "submit" button tosend your entries. To use this form, you must have Microsoft Excel installed on your computer.

Follow these steps carefully:
  1. Click  HERE to access the entry form file.
  2. Choose to "Save to Disk" and save the file to your computer's desktop.
  3. Open the file with Microsoft Excel.
  4. Complete your entries.  For phone numbers, simply enter numbers only without parentheses or dashes.  They'll format automatically.
  5. After you're done, SAVE the completed file to your desktop and CLOSE out of Excel.
  6. Attach the completed file to an email addressed to mick.torres@d128.org (please include your school name in the "Subject" field.)
  7. I will email me you a verification of entries within 48 hours.
