Homebound and Hospital Instruction
Home/hospital services are provided to a student when a licensed medical physician determines that the student will, or is anticipated, due to a medical condition, be out of school for a minimum of two consecutive weeks of school (10 days) or more or on an ongoing intermittent basis. The goal of home/hospital instruction is to afford the student experiences equivalent to those afforded to other students at the same grade level and designed to enable the student to return to the classroom. Thus, the substance or content of the instruction, generally academic, is to enable the student to remain synchronized with the other students in his or her class.
Parents/Guardians
Parent/Guardian Information for Accessing Home and Hospital Instruction (2017-2018)
Your student may be eligible to receive Home or Hospital Instruction Services if he or she is unable to attend school due to a medical condition for: (1) 2 or more consecutive weeks; or (2) on ongoing intermittent basis consisting of anticipated absences of at least 2 days at a time, multiple times during one school year, totaling at least 10 days of absences. Eligibility for Home and Hospital Instruction Services will be determined when all necessary paperwork is completed and returned to your student's school counselor, who will work with the Special Services Department to confirm eligibility and make tutoring arrangements.
Requesting Homebound Tutoring:
The following forms are required to request homebound tutoring:
- Parent Consent - Includes basic student and parent/guardian information and requires a parent/guardian’s signature verifying their request for the student’s participation in Home or Hospital Instruction.
- Medical Certification - Completed by a physician licensed to practice medicine in all its branches (M.D. or D.O.). Must include: diagnosis, types of services prescribed, anticipated length of time the student is expected to be absent from school, doctor’s original signature.
- Release of Confidential Information - This is required by law to allow authorized school personnel to receive any necessary reports or information during the student’s absence from the school.
Returning to School After Extended Homebound Leave:
When your student is cleared to return to school by his/her physician, the school's team can best support his/her reintegration into the learning environment with the provision of the Medical Clearance form. While not required, this form is completed by a physician (M.D. or D.O.) licensed to practice medicine in all its branches and returned to your student's school counselor before the s/he returns to school from full time home or hospital instruction. This form will also allow the student’s IEP or 504 team, and/or LST, the opportunity to review your student’s needs to ensure an appropriate plan of support is put in place upon his/her return to school from full-time home or hospital instruction.
As agreed to on the Parent Consent Form, please be aware of the following:
- Your child may be entitled to a minimum of one full hour (60 minutes) of instruction each day that the District is in session or 5 hours per week. (A physician can certify that the student should receive less than 5 hours per week.)
- If your child is determined eligible, an adult must be present in the home when tutoring takes place; most often, this is a parent or guardian. The homebound instructor may not meet with any student alone, and is directed to leave should an adult who knows the student not be present.
- Once home and hospital instruction begins, your child may return to school only after a physician completes a Medical Clearance Form and that form is returned to the Office of Student Services.

Prospective Homebound Tutors
Prospective Homebound Tutor Information
CHSD 128 is looking for staff members (certified teachers, certified paraprofessionals with teacher certification, and/or certified substitute teachers) who are interested in providing homebound tutoring services for students who are in need of support at very difficult times! By responding to this survey, your name will be added to a list of staff who are willing to be contacted for possible homebound tutoring opportunities when they arise!
Often, homebound tutoring services are provided for students with medical or mental health conditions that prevent them from attending school for extended periods of time OR who have ongoing intermittent absences due to their condition. Homebound tutoring is first offered to the student's course teacher, but if s/he cannot provide the tutoring, then other staff members are contacted to fill the position. Homebound tutors typically meet for an hour per subject per week with a student at a time/place that is agreed upon by the tutor, parent and student. In some circumstances, the student may need more or less tutoring time, which will be reviewed on a case by case situation. While most homebound tutoring situations are scheduled and consistently provided, tutors may find that they need to schedule sessions with the student/parent on a weekly basis of availability.
**Please note, often homebound students experience conditions that cause fluctuations in their availability to engage in active learning, and tutoring times should be scheduled and adjusted compassionately and flexibly with this in mind.
TUTORING DURING SCHOOL HOURS - Homebound tutors can provide such support to students during the school day, as long as it is not during an assigned instructional period for the student or the teacher (i.e., during a lunch period/study hall).
TUTORING OUTSIDE OF SCHOOL HOURS - Homebound tutoring is most typically provided outside of the typical school hours (evenings, weekends). Tutoring can be provided to the student in his/her home with an adult/guardian present, at a public location (library, coffee shop), or via Google Hangout/Skype or other electronic means as agreed upon between the tutor, parent, and student. A tutor should NEVER be alone with a student in any circumstance. There is no mileage reimbursement to/from homebound tutoring sessions.
