Health Services
School nurses are an integral part of the student services teams in District 128. The health offices in each of our schools support students' medical needs and address student injuries and illnesses.
Registration Health Requirements
- 9th grade students are required to submit proof of physical examination and immunizations (by October 15th) and dental exam records (by May 15th) of their 9th grade year. Any 9th grade student not in compliance with the physical and immunization requirements will be excluded from school beginning October 15th until the correct information is submitted to the nurses' office.
- Transfer students must submit proof of physical examinations and immunizations.

Vision and Hearing Screenings
Each school provides screenings for vision and hearing according to state regulations. Specifically, within the high school setting:
- Students receiving special education services are screened annually.
- Transfer students are screened following their enrollment.
- Additional students may be screened in response to parent or teacher referral.
If a student doesn't pass their hearing or vision screening, or if other questions arise, a nurse will notify parents/guardians and may recommend further evaluation.
The parent or legal guardian of a student may object to hearing or vision screening tests for their children on religious grounds. If a religious objection is made, a written and signed statement from the parent or legal guardian detailing such objections must be presented to the school nurse.
