Part 4 - Proof of Residency

We digitally verify residency for all incoming 9th graders who attended Oak Grove, Highland, Rondout and other private feeder schools. 

Only the following situations need to provide additional documentation to verify proof of residency:

 

Incoming 9th Grade families who we were not able to digitally verify residency. These families will be notified in the registration letter that is was sent out and will need to complete this form and upload the documentation.

 

Current LHS Parents/guardians who have moved/changed their address must re-verified by completing this form AND uploading the documentation.

 

 

All proof of residency forms need to be complete by June 29th. 

Your registration is finished if you have: 

  • Completed your students online registration
  • Created a Parent PowerSchool Account (if you didn't already have an existing account)
  • Paid your registration fees via our web store
  • Provided Proof of Residency if required!
  • Incoming freshman, don't forget to turn in your health, dental forms if you haven't already done so! Documents are linked in Part 6.

Families who have completed the registration and residency verification process, including fee payment will be able to pick up student schedules, ID cards, locker,  bus information, and a chromebook (new students and incoming Freshman) at the end of July/beginning of August. Date to be determined.
