Part 5 - Medical Forms Submission

All incoming 9th grade students must have a complete ISBE health physical, dental exam & immunization record on file prior to the first day of school. In addition, any student wishing to play a sport, needs a sports physical on file. Medical forms including physicals, asthma action plans, administration of medication etc can be submitted the following ways:

• Dropped off in our secure lock box located at the Studio Theatre Entrance
