Part 5 - Documents & Submission Instructions
All incoming 9th grade students must have a complete ISBE health physical, dental exam, & immunization record prior to the first day of school. ISBE health/dental/immunization forms, athletic physicals, and paper fee wavier applications can be submitted to VHHS with the following options:
- Upload any health related documents to the following secure file.
- Mail all documentations to VHHS: Vernon Hills High School, Attention: Data Team, 145 North Lakeview Parkway, Vernon Hills, IL 60061.
- Drop off all documentation at VHHS during summer business hours: Monday-Thursday, 7:30 am to 4:00 pm.
- Drop off all documentation at secure 24 hour exterior lock box outside of door 2/bus lane.
