Post-Secondary Transition Planning 

What does post-secondary transition planning look like? 

Post-secondary transition planning for students in Illinois begins in the IEP of the year that the student turns 14 ½.  A student must be invited to any meeting over age 14 ½ years where transition planning will be discussed.  

Transition services means a coordinated set of activities for a child with a disability that:

  1. is designed within a results-oriented process, that is focused on improving the academic and functional achievement of the child with a disability to facilitate movement from school to post school activities, including post-secondary education, vocational education, integrated employment, continuing and adult education, adult services, independent living or community participation…
  2. is based on the individual child’s needs, taking into account the child’s strengths, preferences and interests

Transition planning, as part of the IEP, must include the following components in order to be effective:

  • Transition Assessments
  • Post-Secondary Outcomes
  • Course of Study
  • Transition Services and Linkages
  • Annual IEP Goals

What post-secondary transition services are offered in D128?

Community High School District 128 offers a variety of resources and guidance through each student’s case manager and special services team to support the development of an individualized educational plan that promotes improved outcomes for graduates when exiting high school. Our Transition Coordinators can support each student’s IEP team in the identification of individualized experiences for students to explore and develop their independent living, vocational and academic skills readiness through experiences offered within D128, SEDOL, and the surrounding community.
