SPECIAL SERVICES DEPARTMENT
The mission of Community High School District 128 is to provide quality educational opportunities that foster the development of well-rounded, civic-minded learners and citizens. The special services teams at Libertyville High School and Vernon Hills High School strive to support all students in their high school experience based on their individualized needs and with creative planning. D128 provides a continuum of special education and related services to provide educational opportunities for all learners, including through our collaboration with the Special Education District of Lake County. Services provided through the Special Services Department include Special Education planning under IDEA, 504 Accommodation planning under ADA, and Home/Hospital Instruction services.
NOTICE REGARDING RELATED SERVICE LOGS ACCESS
Students with disabilities may receive related services as part of their individual education programs (IEPs). Lake Forest High School District 115 will maintain related service logs that record the type and number of minutes of the related service(s) administered to such students. Copies of any related service logs will be available to parents/guardians at their child’s annual review IEP meeting. Parents/guardians of students with disabilities may also request copies of their child’s related service logs at any time.
NOTICE REGARDING STUDENTS WITH DISABILITIES: HOUSE BILL 5770
House Bill 5770, which became law on August 28, 2018, requires that, beginning with the 2019-2020 school year, a school board posts on its internet website and incorporates into its student handbook or newsletter notice that students with disabilities who do not qualify for an IEP may qualify for services under Section 504 of the Rehabilitation Act of 1973 if the child: (i) has a physical or mental impairment that substantially limits one or more major life activities; (ii) has a record of a physical or mental impairment; or (iii) is regarded as having a physical or mental impairment.
