Dear Parents/Guardians:

As you are aware, in partnership with Passport Health, District 128 will provide voluntary Pfizer vaccinations to our 16+-year-old students on May 11 & 12, and then again on June 1 & 2. 

In addition, as reported in the media, it is widely anticipated that the FDA will approve the Pfizer vaccine for students 12 - 15 years of age next week. Again, utilizing our partnership with Passport Health, we have been actively planning to voluntarily vaccinate our current LHS - VHHS high school students who meet the 12 - 15 age requirements once FDA approval is announced. 

To that end, we have tentatively scheduled vaccination dates at LHS and VHHS for those vaccinations; however, the tentative dates are dependent on FDA Pfizer 12 - 15 vaccine approval and may be subject to change as a result.

Libertyville D70 is facilitating vaccination for its current, eligible students as well as for current, eligible students at Oak Grove D68 and Rondout D72. D73 is also facilitating vaccination of its current, eligible students.

The D128 vaccination dates for current LHS and VHHS students are:

VHHS - Monday, May 17

LHS - Thursday, May 20

Dose 2

VHHS - Monday, June 7

LHS - Thursday, June 10
 

Once FDA announces approval of the Pfizer 12 -15 vaccinations, you will receive specific information from LHS or VHHS via e-mail regarding the vaccination process, registration and scheduling. 

The vaccine is free to all D128 students meeting age requirements, and there is no cost to the district to administer the vaccine. As with all COVID-19 vaccine programs, parents-students with insurance will be asked to submit insurance information to offset vaccine administration costs. As with all vaccination programs, the LHS - VHHS vaccination programs are subject to vaccine availability from CDC - IDPH - LCHD distribution pipeline. 

Our sincere appreciation and thanks to the D128 Board for its support of this important initiative, to Passport Health for its continuing partnership with D128, and for its work with the Lake County Health Department to make this initiative a reality. In addition, special thanks to Ole Stevens (LHS) and Andy Young (VHHS) for their herculean efforts in working with us to plan, facilitate and coordinate this important vaccination effort.
