D128 Parents/Guardians:

The Lake County Health Department requires a COVID-19 test for any student or staff member that is symptomatic, or an alternative diagnosis from a doctor, before returning to school. This applies to both vaccinated and unvaccinated individuals. Your student’s school must receive a copy of the negative test result before returning to school. We cannot accept home test results. If the test is positive, please contact the nurse's office as soon as possible.  

When reporting a student’s absence due to illness, please share with the LST, any symptoms and if you are planning to have the student tested for COVID-19. If no information is given, you will receive a follow-up phone call either from the LST or nurse’s office asking for this information.

Testing options

For testing options, first contact your doctor or pediatrician’s office. Refer to this list for other local testing sites.  Please email or fax a copy of the test result to your student’s school nurses’ office:

If a student or staff member has symptoms only and their COVID test is negative, they should stay home until symptoms are improving or not worsening. If they have a fever, they should be fever-free for 24 hours before returning to school. Once the school receives the test result or doctor’s note, the student’s absence may be changed from an excused absence (EA) to a medical absence (ME).

 
As a reminder, COVID Symptoms are:  

  • Fever (temperature =/> 100.4)  -  STAY HOME
  • Cough
  • Shortness of breath or difficulty breathing
  • Muscle or body aches, chills
  • Sore throat
  • New loss of taste or smell
  • Headache
  • Diarrhea, Nausea, or vomiting  (if stomach symptoms only, don’t need a test)
  • Congestion or runny nose  (that is not caused by recent, known allergy symptoms - it must be a NEW onset or recent start of this symptom)
